Update branding assets and dynamic document titles: 

- Replace placeholder icons with new SVG assets (`favicon.svg`, `logo-mark.svg`, `pwa-maskable.svg`).
- Update document titles dynamically based on the active page.
- Customize headers and sidebars with `logo-mark.svg` for a consistent branding experience.
- Adjust index title to "Ban Guardian".
